History

The Apollo RCA method was a direct response to the challenges faced by industry in the early 1980s - high incident rates, complex problems, and significant consequences.  Aiming to provide industry with a simply applied, yet effective, problem-solving tool, the Apollo method was introduced in the mid-1980s by Dean Gano, author of "Apollo Root Cause Analysis - Effective Solutions to Everyday Problems Every Time."  Each year since then, thousands of students worldwide have participated in Apollo training.
